Death can look however she wants. Though this is highly unlikely as they can only be together if Deadpool dies which he cannot do. (Due to in part his powers and the curse)
If it's real it's probably Outlaw aka Inez Temple which would be lame. Siryn would be the preferred choice since she's on the short list of girl's he's banged recently who's still got a pulse. Both of these two guesses fit the picture.
