Dead Space got to me a lot for about the first half of the game, I was very tense and jumpy while playing.
Doom 3 was pretty tense for me at first too. I think overall, though, the scariest play experience I've had was the single player in AvP2 on PC.

This. That game just cultivated an ambiance of jitteryness from the musac/sound effects of the whispering and creepy environmental sounds to the random necromorphs that would just be there when you turned around.
On a personal note, the first game that ever scared the shit out of me was Michael Jackson's Moonwalker for Sega Genesis. I still wake up in cold sweats every now and then from nightmares about that game.
