I had the same problem this morning. Go back and play any chapter from any episode by yourself on easy, or whatever difficulty you want. For some reason this has been unlocking it. I searched forums this morning and that was the answer that was being given. I played back through the very first chapter of episode one on easy. Once I finished it the achievement popped.

Have 3 people sign in guests to fill a CTF lobby. Get achievements. There's no banhammer in H4. You can boost/cheat/mod and never get penalized.
