10th Prestige secret achievement from Cod World At War
There are a few more that were completely brutal to obtain, but I can't think of more off the top of my head...The GTA IV one was by far the worst because of the fact that my save file corrupted when I had one pigeon left, so I had to rebeat the entire game up to 100% again =\

My top 5 (in no peticular order)
1.) Score Gold Medal (Tetris Evolution)- Score 25,000 points in 4:00 while playing score mode.
2.) Turtle Soup (TMNT Arcade)- Defeat Shredder without being turned into a regular turtle.
3.) Kick The bucket Award (Guitar Hero 2)- Complete Jordan on Expert.
4.) Mile High Club (CoD: MW)- Skydive to safety on Veteran Difficulty.
5.) Complete Package (The Simpson's Game)- Finish every episode, find every collectible, and discover every video game cliche'.
i'm absolutely sure there are harder ones, but out of what i have played, these are tops at the moment.
My top 5 (in no peticular order)

Demon on Wheels in Trials HD was a bitch. Completing all Extreme tracks.
Also, The One Free Bullet achievement in Half Life 2 episode 1, that was was but challenging, as well as Little Rocket Man in episode 2, getting that little gnome to the rocket ship us super annoying but really rewarding when you accomplish it.
