There are five difficulty modes (easy, normal, hard, chaos, and hell). It doesn't start getting tough until the last two modes. The trick then is to not rush, just take it slow and kill the enemies from a distance if you can. In those modes, it only takes a few shots for enemies to kill you. The one good thing is that once you're playing the last couple of playthroughs, you've got a pretty good idea where all the enemies are, so you can anticipate them. It's not that great of a game, and having to play through it five times doesn't make it any better.
