There are so many for this list. For me there are two in every game. 1 you are proud of and 1 is the most hardest ever. Some are the same. Like for COD 4 it was mile high proud and hard.
Mass effect 2 insane was easy for me.
But he is one of the hardest achievements ever. Dragon age Golums Beat it on nightmare or the other difficulty. It sat there being the last achievement for 6-7 months. I tried for a long time. No matter how a played i lost. Until i leveled up my guy with hundreds of tomes. All 3 companions died half way into the 1st fight. But my guy beat it all by his lonesome.
Getting that achievement was like winning an Olympic gold medal.

Best three games ever. Have all achievements, except DA2 DLC

Mine would be the achievement for beating Devil May Cry 4 on all difficulties. DMC4 was a really challenging game, and it took me forever to beat Dante Must Die mode.

One Great Career in nhl 2012,not hard as in getting it but it was really hard going through the 160 82 games it took me to score all the points,it really put me off playing the game all together.

In modern warfare 2 queen takes rook was really annoying I didn't have alot of trouble with beating the enemy of my enemy mission but loose ends on veteran was terrible, i raged alot due to dying as soon i respawned at my previous checkpoint
