found this off of x360a, and i must say it works quite well.
*spoilers if you havent finished, but i dont think thats very many*
[code=goothgone]Beat the game on any difficulty (i beat it on hardcore) then you unlock Insane difficulty now here the trick *go to co-op campaign split-screen or online (i choose split-screen) choose the same save that you beat the game now pick Act 1 Chapter 1 then for your main profile choose Insane difficulty and for the 2nd controller take Casual now just take the 2nd controller and play the game you can run in all enemies like rambo the enemies are a joke it took me 5-6 hours just take the 1st controller to take cover if you want.* achievement unlock 240GS ENJOY
The profile on Casual will only get the Casual achievement he will not receive the Insane achievement but will get all the others achievements related.
The part where you split-up doesn't matters.
Sometimes you will need to use the first controller(insane), i probly kills 10-15 enemies the whole game.
ACT 2 CHAPTER 6: Run for 15-20 yards with one controller stop then take the other controller do the same thing, rinse and repeat to the checkpoint.
ACT 3 CHAPTER 1: When you carry the bomb do not shoot the enemies melee them press B on both controller

boss guide put the insane controller in front of the boat he will not get hit at all in the boss fight only use the controller on casual to fight the boss[/code]
Parts easily done Solo:
-Riding the Brumak (act 5, chap 5)
-Inside the Riftworm (act 2, chap 6)
-Driving the Centaur (act 3, chapter 3 i think it is, not confirmed)
-Facing Skorge's Hydra (not confirmed by me, but Aldemar says it is. will get back to this later on today)
-Getting out of the Burning Building (act 5, chap 4, just before you get in the elevator and everything is burning)
