CoD2 Corn silo with a check point with a mortor coming down on you sucked it happened to me two times in a row. Became pretty good at clearing those houses.

I found any COD game on Vet hard. I found NFitWR MUCH easier than Mile High Club, I guess it depends on your style of gameplay 
I spent 8+ hours on MHC. Grenade rain on COD:W@W = major fail. I'm sure the armies had tons of extra grenades to throw around like that.
The single hardest game to 1k (that I have done) IMO is Facebreaker. According to mygamercard.net I am one of only 54 people to have done so. The game has been out since last year august/september time frame

hmmm...i havent played TOO many that were overly difficult...but there are some achievements specifically that i find hard to get
beating BAL on hard on def jam: icon. omg hard is so...insanely difficult its not even funny. takes forever to win one match...
beatin dishwasher on samurai (aka the hardest) mode. im having trouble beatin the last dude on normal, much less that!
beatin prototype on hard. my first playthrough was on easy, and i had a few probs every now and then (usually when im tryin to attack someone/thing and those hunters or super soldiers keep wailin on you and you cant get out of the way!)
just a few offhand..

Devil May Cry 4 was up there, Street Fighter IV as well. Far Cry Instincts Predator, because the game is soooo bad it's impossible to get a 1000.

I would have to say that in my experience, Blue Dragon is probably the worst I have seen for achievements. I know Minioger has the patience of a saint to have all 1000 points on that one. I have about 5-6 hours in and I think 5 points to show for it, 10 at most.
I wanted so bad to get into that game, but it just never delivered. How anyone could push through the beginning of that one and keep going for literally hundreds of hours, is beyond me! Much props Mini!

I only had about 110 hours to 1k the game, but I did it all on one playthrough. Plus I loved the game and even have Blue Dragon Plus on the DS because I loved the story so much. Really didn't require patience on my part. But I guess it depends on how much you like a game.
