Fallout difficulty

I've recently started a new Fallout 3 game, but this time I wanted to experience it as though surviving is much much harder, so I placed the difficulty to very hard, but I soon realized that the game doesn't really force me to do desperate things to survive, i.e. eat people or flee.

well boobs there is a perk you can have that you can eat corpses, but just because you change the difficulty to very hard doesn't mean that you automatically have that perk enabled into you.....
doesnt matter to me really, i put it on very easy just because lol i thought about making it higher but having it on very easy and being able to own (just about) everything is awesome lol

i eat dead people
i tried the game on hard, and as far as i can tell enemies hurt you alot more. that's about it. maybe they take longer to kill? probably, but i didn't try it for too long lol.

I had it on Hard, but then i set it back to Easy. Hard is hard enough and ammo consuming.
