I don't know the order of the ranks, but if you look at the bar in your character selection, it fills up from left to right. Each black bar is a new rank. I didn't even pay attention to the ranks until I was 4th or 5th from the top and it still had some sort of insulting rank name...lol
Once you hit the third bar down you will get the achievement for that rank....etc
