Upgrade to Elite or 120GB drive and need the cable?

For those of you that did or are thinking of upgrading to the Xbox Elite or the 120GB HDD, there is a form you need to fill out to get a free transfer cable sent to you. This should have been in the boxes, but at least it is being shipped at no charge.
