Adjust the sliders so that penalty shot is full and everything else is down, the take a hot from the blue line while the defender hits you, it might take a few times, but it will work.

I feel your pain Knuck I cannot get the other team to commit a penalty shot either. I'll try it with two controllers later tonight.

Here is how I got it. I posted this on another site and have had a lot of people tell me they got it on their first try this way. It's long, but here goes....
This is by far the easiest way to get this achievement. I tried the other ways and after about 30 mins couldn't even get a penalty shot. I tried this method and got the ach on my first try!!
1. You need 4 controllers (any mix of controllers and guitars will do).
2. One controller on the team to get the achievement and three on the other team.
3. Put penalty shot slider all the way up.
4. Commit 2 penalties with the team that has 3 controllers. You are left with one player for each controller.
5. Ensure all 3 players are outside of their blue line. None of them will move since they are user controlled.
6. Bring the puck past the other teams blue line with any player you choose from the team you want to get the penalty shot with. You are now on a breakaway.
7. Once you are there hold the X button down.
8. Bring a player from the team with 3 controllers on it down and commit a penalty on the player with the puck by using the right analog stick. You should get a penalty shot.
9. Push down the right stick so you can control the goalie, then move him to one side of the net. Now with the other team, simply go down and score on the empty side of the net.
I hope this helps anyone out that is trying to get this.

It took me almost an hour to get this one. I played against the computer and I turned the penalty slider all the way up. I played with a really good team (don't ask me who) and then basically I constantly stole the puck and tried to make a fast break and would just mash the button a lot to shoot (b maybe?) so that I would wind up to shoot but not actually make the shot, frequently I would get hit by the computer but no penalty shot, eventually the computer had so many people in the box it eventually worked. I haven't tried any other method so mine may not be the best, but it worked for me (after a while)
